Facility Evaluation Report
On March 13, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case management visit in relation to complaint control: 14-AS-20260227122349. LPA met with Resident Services Director, Anne Dasmarinas and explained the purpose of the visit.
During a complaint investigation, LPA requested to review personnel training records, however the facility was unable to provide LPA training records for the requested staff members.
Deficiency was observed during the visit and cited from the California Code of Regulations, Title 22 and Health and Safety Code. See LIC809-D.
Report is reviewed with Resident Services Director, Anne Dasmarinas and a copy is provided with appeal rights.
