Report a correction
What can be corrected?
Anything we have got wrong in how we present the state’s record — a mismatched facility, a wrong address, a misread capacity, a figure that does not match the state source. We will correct it and say when we did.
What we can and cannot change

| Our page misreports the state record | We fix it, usually within two working days, and note the correction. |
| The state record itself is wrong | We cannot change a state record. Correct it with the licensing office and the change flows through on the next refresh. We will publish your response in the meantime. |
| A citation is old, or was resolved | It stays on the page, because it stays on the state record. You can add a response explaining what changed, shown directly beneath it. |
| Remove my facility entirely | No. These are public licensing records about a licensed business, and the point of this site is that the record cannot be edited by the people it describes. |
How to reach us
Email corrections@seniorcarerecords.org with the facility name, its licence number and what is wrong. Include the state record you are relying on if you have it.
Senior Care Records is published by MITRYXA. Corrections are handled by a person, and we will tell you what we did either way.
