The inspector’s account

On March 5, 2026, Licensing Program Analyst (LPA) Komal Curley conducted an unannounced case management visit. LPA met with Resident Services Director, Anne Dasmarinas and explained the purpose of the visit.

On 3/1/26, LPA was made aware via email of an eviction status for Resident 1 (R1). On 3/2/26, LPA emailed the Special Operations Director asking when the eviction notice was issued to the resident as based on LPA's records review, CCLD did not receive a copy of the eviction notice provided to R1. According to the Special Operations Director, the eviction is in collections and with the facilities legal department. Special Operations Director provided LPA with a copy of the unlawful detainer. LPA reviewed the document and observed a copy of a 30-days eviction notice signed by Staff 1 (S1) dated 10/28/25 addressed to R1 for nonpayment of the rate for basic services.

During the visit, LPA reviewed documents. S1 no longer works at the community so LPA was unable to confirm if the eviction notice for R1 was sent to CCLD or not. The facility was unable to provide documentation to show that a copy of the 30-day eviction was submitted to CCLD. LPA reviewed CCLD records and was unable to find a copy of the 30-day eviction notice.

Deficiency was observed during the visit and cited from the California Code of Regulations, Title 22 and Health and Safety Code. See LIC809-D. Report is reviewed with Resident Services Director, Anne Dasmarinas and a copy is provided with appeal rights.
