Facility Evaluation Report

On 5/27/2026 at 5:30 pm, Licensing Program Analysts (LPAs) Y. Brown and K. Nguyen arrived unannounced to conduct a Case Management visit. LPA met with Administrator and explained the purpose of the visit.

While LPA Y. Brown was conducting a complaint investigation (#15-AS-20260319151737) on 5/27/2026 during facility tour and interview LPA observed:

1. S1 was not fingerprint cleared and associated to the facility.

The deficiency was observed (see LIC809D) and cited from the California Code of Regulation, Title 22. Failure to correct the deficiencies may result in civil penalties.
