On 9/19/2025 at 11:00 AM, Licensing Program Analyst (LPAs) Y. Brown and J. Sampair conducted a case management-legal non-compliance visit and met with Licensee Ericka Tillis. Upon entry, the LPAs explained the purpose of the visit.
During the visit, LPAs and Licensee discussed the update on the relocation of the residents. The LPAs observed three (3) residents at the facility and a fourth (R1) is in the hospital.
