On 9/5/2025 at 8:00 AM, Licensing Program Analyst (LPAs) Y. Brown and J. Sampair conducted a case management-legal non-compliance visit and met with Licensee Ericka Tillis and LPAs explained the purpose of the visit.
During the visit, LPAs and Licensee discussed the update on the relocation of the residents, the residents ambulatory status and the total census at the facility. Licensee stated that they have been working with the residents responsible parties and Public Guardian to relocate the residents.
Exit interview conducted and a copy of this report provided.
