On 8/04/2025, at 12:45 PM, Licensing Program Analyst (LPAs) James Sampair and Yasamin Brown arrived unannounced to conduct a Plan of Correction (POC) visit. Upon arrival, the LPAs stated the purpose of the visit to Licensee Ericka Tillis.
LPAs toured the facility, including the residents' bedrooms, bathrooms, common area, kitchen, and outdoor areas. 7 days of non-perishable and 2 days of perishable food supplies were present.
Exit interview conducted and a copy of this report provided.
