On 8/04/2025, at 12:45 PM, Licensing Program Analyst (LPAs) James Sampair and Yasamin Brown arrived unannounced to conduct a Plan of Correction (POC) visit. Upon arrival, the LPAs stated the purpose of the visit to Licensee Ericka Tillis.
On 7/18/2025, LPAs Brown and Luk conducted a Case Management visit during which 1 deficiency was cited. The 1 deficiency was cleared during this visit.
