Facility Evaluation Report

On 1/30/24 at 3:15 p.m., Licensing Program Analyst (LPA) Greg Clark conducted an unannounced case management visit as a result of this facility receiving residents from Vista Terrace of Belmont (VTB). LPA met with Stephanie Brice, Administrator (ADM) and explained the purpose of the visit. There are currently 2 residents from VTB remaining at this facility.

ADM stated that R1 and R5 decided to stay at this facility.

During the visit, LPA toured the facility including but not limited to the apartments where the residents from VTB resided (R2, R3, R4, R6 and R7). LPA observed that all the apartments were empty.

There were no health/safety concerns during today's visit.

No deficiencies cited during visit. Exit interview conducted and copy of this report provided.
