On 11/19/2025 at 8:16 AM , LPA conducted a second attempt to complete the closure visit for this facility. LPA contacted the licensee, Ericka Tillis , who stated that she is not available and can not attend the visit.
LPA conducted an external observation of the building, including windows, and observed no residents present in the home.
A forfeiture letter will be mailed to the licensee at a later time.
