Facility Evaluation Report

While investigating a complaint (15-AS-20230302160451) and upon review of incident reports submitted by the administrator, Licensing Program Analyst (LPA) Delmundo observed no report submitted for the incident that happened on February 15, 2023 for resident (R1); nor call was made to Community Care Licensing.

Deficiency is cited from Title 22 California Code of Regulations (see 809D). Failure to submit proof of correction by plan of correction due date may result in civil penalty.

Deficiency and plan and proof of corrections were discussed with Alanna Spencer.

Exit interview conducted. Appeal Rights, LIC9098 Proof of Correction form and copy of this report provided.
