Facility Evaluation Report

At 2:00 pm on this day, January 8, 2026, a virtual meeting was called. The meeting was attended by the following:

Licensing Program Analyst Alicia Delmundo

1. Termination of Lease Agreement and transfer of residents.

2. Submission of corrected facility sketch for the other facility the licensee is applying for license and possible transfer of Gentle Heart Care Services' 4 residents to that other facility and placement of 5th resident to a different facility.

3. Submission of outstanding proof of corrections.

4. Provision of updates to LPA and LPM on transfer/move out of residents by January 24, 2026.
