The inspector’s account

Licensing Program Analysts (LPA) Zabel Chochian conduct a required annual visit. Upon arrival LPA observed two staff and six residents present. The LPA was greeted by staff Paula Trejo Soubai and Bertha Trejo. Reason for the visit was explained. Administrator Mike Trejo arrived shortly after.

At approximately 3:20 p.m. the LPA conducted a tour of the physical plant with staff. Facility is a single-story home that consists of four (4) resident bedrooms and three (3) bathrooms. The LPA observed (2) fire extinguishers last serviced on 06/30/2025. All smoke alarms and carbon monoxide detector were tested and functioned properly during time of visit. The LPA observed all required postings throughout the facility. The auditory alarms on the exit doors were tested and functioned properly at the time of visit. Kitchen : The kitchen appeared clean and the appliances and fixtures observed functional. A seven day non-perishable and two day perishable food supply observed during todays visit. Knives and cleaning supplies observed stored inaccessible. Laundry area is near the kitchen and is kept locked at all times. Bedrooms: The resident bedrooms were properly furnished with at least one chair, nightstand and sufficient lighting for each resident. The bedrooms had appropriate and adequate bedding and linens such as sheets, pillowcases, mattress pads, and blankets. Bathrooms: LPA observed restrooms to be properly supplied and had functional fixtures. Hot water measured at 110.1 degrees Fahrenheit in all restrooms. Common Areas: These included the living room and dining area. The common areas were checked for cleanliness and furniture was checked for functionality during time of visit. The facility maintained a comfortable temperature of 78 degrees. Surrounding Grounds (Outdoors) : There was a shaded area with proper furniture for outdoor use. The LPA observed a swimming pool properly gated, locked, and inaccessible to residents in care. Garage: Garage access is only from the outside and used as storage. Due to time constraints the annual visit will continue on a later date. Exit interview conducted and copy of the report provided.
