Facility Evaluation Report
Licensing Program Analyst (LPA) Elsie Campos initiated a Case management incident visit to the facility above. As a result of an Incident Report (IR) that was received on 7/21/2023. The LPA met with Staff Christopher Dike. Administrator Maria Lassiter arrived shortly thereafter. The LPA subsequently conducted a visit regarding Complaint #29-AS-20230721124526.
During today’s visit, the LPA completed a physical plant tour with Staff no immediate health and safety concerns were observed during today’s visit. The LPA was accompanied by Ventura County Sheriff Officer Scott for the initial part of the visit. LPA obtained pertinent documents.
On 7/21/2023, the facility submitted a TRI-Counties regional center vendor incident report (IR) , alleging that on 7/20/23, Resident #1 (R1) reported that they were sexually assaulted by Resident #2 (R2). It was alleged that R2 went into R1's room, R2 took off their clothes and laid down in the bed with R1 and proceeded to perform a sexual act on themselves and subsequently perform a sexual act on R1. R1 mentioned the assault happened during the 6am-2pm shift and that Staff #1 (S1) had witnessed R2 coming out of R1's room after the assault occurred. R1 stated they did not tell staff at the time and kept the incident to themselves due to language barriers . In response, the facility notified the responsible parties for R1 and R2, called the Thousand Oaks Sheriff’s Department, and reported the incident to the California Department of Developmental Services and Community Care Licensing.
Further investigation is required prior to issuing findings. Exit interview conducted. A copy of the report was issued. The LPA requested pertinent documents to be emailed to the Department within the next 24-48 hours
