Facility Evaluation Report
Licensing Program Analyst (LPA) KaSandra Lopez conducted an unannounced Case Management Deficiencies inspection due to a deficiency observed during a complaint investigation. The LPA initially met with Leticia Higares at 9:55 AM. The visit was concluded with Director of Health and Wellness Ian Gadea.
During record review during a complaint inspection, the LPA observed Resident #1 (R1) to have a diagnosis of Dementia and their medical assessment on file is older than one year (dated 07/28/2020).
The following deficiency was observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22 and/or California Health and Safety Code. Failure to correct the deficiencies may result in civil penalties. Exit interview and report reviewed with Ian Gadea. A copy of the report and appeal rights were provided.
