Complaint Investigation Report
Interviews during this investigation with Jacob Primeau and during a previous investigation under complaint control # 29-AS-20210610131205 with Business Office Manager Anabel Amaya on 06/14/2021 revealed, the Administrator at the time Martha Berard was on leave but the facility had interim Administrators and multiple staff from their Regional Office to provide administrative support for the facility. Also at the time the complaint was filed, reception did not have access to the resident roster and it had to be printed by the Business Office Manager, which it was done during the complainant's visit. Based on the information obtained, there is insufficient evidence to support the allegation occurred. Therefore, the allegation of 'Insufficient staffing' is deemed Unsubstantiated at this time.
