The inspector’s account

Licensing Program Analysts (LPA) Esther Cortez conducted an unannounced Case Management - Deficiencies visit in conjunction with a complaint visit (Complaint control # 29-AS-20250618161202 ). The purpose of this visit is to issue citations for deficiencies observed during the complaint investigation which were not related to the complaint. Upon arrival LPA met with Senior Director, John Lynch and explained the reason for the visit.

Prior to visit, the LPA printed out the facility personnel report summary from the Licensing Information System (LIS) which revealed that four (4) staff that are on the staff roster provided to the LPA during the visit were not associated to the facility as they were not listed on the LIS report. The LPA reviewed the Guardian website and discovered that all four staff had fingerprint background clearance however were not associated with the facility. Per the Senior Director, two of the staff were present at the facility during the visit and the other two were off. Furthermore, facility HR personnel confirmed all four staff were not associated to the facility and upon observation had them associated.

Pursuant to Title 22 of the California Code of Regulations Division 6, Chapter 8, the following deficiencies were cited (refer to LIC 809-D). Civil penalties assessed in the amount of $1,600. Failure to correct the deficiencies may result in additional civil penalties.
