The inspector’s account

Licensing Program Analyst (LPA) Zabel Chochian conducted a required annual visit to this facility. Upon arrival LPA met with facility's Senior Director John Lynch.

A physical plant tour was conducted upon arrival with Mr. Lynch. The facility is a 2 (two) story building located in a commercial area in Thousand Oaks, CA. Fire Cleared for a maximum capacity of 100 (one hundred) non-ambulatory clients (approved for first floor only). All combination carbon monoxide/smoke detectors as well as sprinkler system are functioning/operable and maintained by Ever On company. Several fire extinguishers observed through out the building were inspected on 1/06/2025. The program is structured towards each person’s Individualized Support Plan (ISP). The facility has dedicated nursing staff on site, to allow for clients of varying health needs accessibility to programs offered. The facility consists of various art studios, performing arts spaces, a kitchen, fully accessible restrooms for clients and staff, changing areas including, exercise room, health clinic, media center, gathering space, basketball courts. All rooms identified contain locked inaccessible storage space. Sharps are kept in a locked drawer in the program building with no access to clients. All other areas observed appeared to be safe and free of hazards. The bathrooms are safe and sanitary and in operating condition with toilet paper, soap, and paper towels. Participants bring their meals and snacks. A variety of emergency safety drills and a pandemic procedure review are conducted monthly. Emergency Disaster and Infection Control plan observed during today's visit.

Centrally stored medications are stored inaccessible to clients and others. Medication policy and procedures reviewed with facility LVN. Six (6) staff records were reviewed and found to be in good order.

Due to technical issues and time constraints annual visit will be continued on a later date. No health and safety issues observed during this visit. Exit interview conducted and copy of report provided.
