Facility Evaluation Report
At approximately 1:15 p.m. Licensing Program Analyst (LPA) Erica Mosley conducted a Case Management - Deficiencies visit due to a deficiency observed during a complaint investigation.
At 1:40 p.m. LPA requested to review former resident files that have resided at the facility within the last 3 years. LPA was informed telephonically by the Administrator that the files were not available however the Administrator could fax them to the LPA by the end of the day.
The following deficiencies were observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22 and California Health and Safety Code. The Licensee was made aware that failure to correct the deficiencies may result in civil penalties. Exit interview conducted. A copy of the report and appeal rights were provided.
