Facility Evaluation Report

Licensing Program Analyst (LPA) Erica Mosley conducted an unannounced Case Management – Other inspection at the facility today due to the closure of the licensed facility. Today, October 9th, 2025, at 9:40 a.m. LPA Mosley was greeted by Wilfredo Macandili who called the Administrator. The Administrator is unable to physically attend and designated her husband, Wilfredo Macandili to sign the report on their behalf. The Administrator was available telephonically throughout the visit.

On September 19 th , 2025, the Department was advised by Maria Macandili, Facility Administrator / Licensee Representative that after careful consideration and due to personal health reasons to close the facility and surrender their RCFE License. As of 9/11/2025 their last resident moved out.

Today’s inspection is to ensure that there are no longer any elements of care or supervision of a licensed facility being provided at this location and to officially close the facility. At 9:45 a.m., LPA Mosley and Wilfredo Macandili conducted a brief tour of the home. During the inspection, the LPA found no evidence to support that the home is being operated as a licensed facility. The license was forfeited at the time of the visit. Exit interview conducted and a copy of the report was provided.
