The inspector’s account

Licensing Program Analyst (LPA) Martha Arroyo arrived at the facility unannounced to conduct the required annual visit today. Upon arrival, the LPA was greeted by staff, Adolfo Gandia who then contacted the Administrator telephonically and informed them of the visit. The Administrator, Walfre Alvizures was unavailable to meet at the facility but gave permission for staff to assist LPA and sign report. Entrance interview conducted.

Beginning at 09:05am, the LPA along with staff toured the physical plant areas inside and outside to ensure there are no health and safety hazards, and facility is in compliance with Title 22 Regulations. The following was observed:

Bedrooms: There are five (5) bedrooms for resident use. Four (4) bedrooms are designated as private / single bedrooms, and one (1) bedroom is designated as shared / double occupancy. Bedrooms were observed to be furnished appropriately and had sufficient lighting. There is a closet with additional clean linens and towels for resident use. There is a staff bedroom on premises which was observed inaccessible to residents at the time of the visit.

Restrooms: There are three (3) restroom for resident use. Bathrooms were clean and sanitary and in operating condition with non-skid surfaces and grab bars. The bathrooms were sufficiently stocked with supplies and paper towels. Starting at 09:08am, the hot water temperature was measured in resident bathrooms, and they measured 112.1 and 113.5 degrees Fahrenheit, which is within the required range of 105 – 120 degrees Fahrenheit. Personal hygiene items were observed inaccessible to residents at the time of the visit.
