Licensing Program Analyst (LPA) Brian Balisi attempted to conduct an unannounced required annual at facility. LPA arrived at facility and awaited at the front door. Licensee Lisa Graham contacted LPA and stated they were out of the city at the moment and were not aware of what time they will return.
LPA will attempt another visit on a later time.
LPA emailed a copy of this report to licensee.
