The inspector’s account

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit at approx 09:50 a.m. for the purpose of investigating self-reported incident reports. LPA was joined by Tri-Counties Regional Center (TCRC) Quality Assurance Specialist (QAS) Patrick Brown. Upon arrival LPA met with House Manager Kassim Adelke and explained the reason for the visit.

On 04/06/2026, the Regional Office received a self-reported incident which stated, on 04/04/2026, at approx 02:30 p.m. the family / responsible party of Resident #1 (R1) reported to the administrator that R1 informed them their "butt hurt and Staff #1 (S1) raped them". R1 was admitted into the Emergency Room as a precaution. At the hospital R1 met with officers from the local police department. R1 stated to the police officers that R1 made a false allegation and no harm was done to R1.

At approx 09:55am. LPA and QAS conducted physical plant, interviewed staff, clients and reviewed and obtained copies of pertinent documentation relevant to the investigation.

Interviews and records review revealed S1 was not on shift the day of the alleged incident. Interview with R1 revealed they feel safe and comfortable residing in this home and have no immediate health and safety concerns at this time. S1 is on an administrative leave at this time.

No health and safety concerns were observed during the visit. LPA and QAS has determined no further investigation is needed at this time.
