Facility Evaluation Report
Licensing Program Analyst (LPA) Zabel Chochian arrived at the facility unannounced to conduct a required. The LPA met with Ash Galardi and staff. Reason for the visit was stated. There were fourty one(41) consumers and ten (10) program staff present today. The program currently operates from 8:00 a.m. to 2:00 p.m. The day program was staffed with 1:3 and 1:1 staff to consumer ratios. Temperatures of staff and consumers are taken upon entry into the facility and at the end of program.
Common Activity Space : The facility is a single-story structure with a large activity room, small conference rooms, two (2) bathrooms, offices, and a food service area. LPA and Administrator toured the physical plat areas inside and outside. LPA did not observe any obstructions or hazards. Furniture and equipment was observed to be in good condition. Smoke detectors and carbon monoxide detectors were in operable condition. Activities: Activities are both designed for individual and as a group. The LPA observed staff working with consumers. Food Service : The kitchen area was clean and in good condition. Consumers bring their lunch and snacks. Consumers also go out to eat. Restrooms : Restrooms observed to be clean and sanitary and stocked with soap, paper towels and toilet paper. Staff and client files reviewed from approximately 11am-12pm; LPA checked staff associations and identified that all staff were fingerprint cleared and associated to this location; staff training and all required documents observed on file. Client records reviewed and also observed to be in order with required documents. Medications observed stored/locked in the Administrator office. Current facility is storing and administering prescribed medication for three (3) clients. Medication record maintained for the three (3) clients.
Facility has a sufficient supply of Personal Protection Equipment (PPE). The facility’s cleaning protocol observed to be sufficient. The facility's procedures as it pertains to infection control were adequate.
No deficiencies cited at this time. Exit interview conducted. A copy of the report was issued.
