Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit at for the purpose of investigating self reported incident report. Upon arrival, LPA met with Licensee and explained the reason for the visit.
On 02/17/2026, Administrator received information that Client #1 (C1) was allegedly pushed by staff and fell to the ground.
At approx 12:30 pm. LPA conducted physical plant, interviewed staff, clients and reviewed and obtained copies of pertinent documentation relevant to the investigation.
Based on interviews and records review no immediate or potential health and safety concerns were observed during the visit and no deficiencies cited at this time.
LPA has determined further investigation is needed and will return at a later date to complete the investigation if warranted.
