Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit at approx 10:00 a.m. for the purpose of investigating a self reported incident report. UUpon arrival LPA met with facility staff and explained the reason for the visit. Facility Administrator Emmanuel Kangala arrived shortly after.
On 11/04/2025, the Regional office received a self-reported incident report, which stated, on 11/04/2025, at approx 4pm, Client #2 (C2) asked for privacy to speak with their family member on the phone in the backyard. Then at approx 05:00pm the family member contacted facility staff stating that C2 needed assistance to return to the facility. When staff checked the backyard they discovered that C2 had exited the property and was found in front of driveway. Staff escorted C2 back to the facility safely.
LPA conducted physical plant, interviewed staff, and reviewed and obtained copies of pertinent documentation relevant to the incident.
No immediate health and safety concerns were observed during the visit. LPA has determined further investigation is needed and will return at a later date to complete the investigation if warranted.
Exit interview conducted. A copy of the report was issued.
