Facility Evaluation Report

Licensing Program Analyst (LPA) Martha Arroyo conducted an announced Case Management-Other visit to the above facility. The LPA met with Administrator, Norma Zanders as the purpose of the visit was to conduct a final walk-through of the facility prior to closure. Entrance interview conducted.

On 04/23/2025, the Administrator contacted LPA Arroyo informing them of the intent to close the facility. The Administrator emailed the Department a copy of the 60-day eviction notice issued to residents and their POA’s / family members. All four (4) residents have been relocated to other licensed facilities. Resident’s information has been inputted in the facility’s closure spreadsheet.

During today’s visit, the LPA conducted a plant tour and observed facility to be vacant and verified that no care and supervision was being provided. The facility will be closed effective June 4, 2025, in the Licensing Information System (LIS).
