Facility Evaluation Report
Licensing Program Analyst (LPA) Quoc Huynh arrived at the facility unannounced to conduct a required annual inspection. LPA arrived at the facility at 9:32AM, met with the Licensee Amelia Banas and explained the purpose of the visit. Entrance interview conducted.
LPA Huynh and the Licensee conducted a tour of the physical plant at 10:26AM to ensure there are no health and safety hazards, and the facility is in compliance with Title 22 Regulations. The following was observed:
COMMON AREAS: At the time of the visit, living room and dining room furniture was observed to be in good condition . An office area was located within the living room and had required postings along the wall. The facility maintained a comfortable temperature throughout the visit. Smoke and carbon monoxide detectors were tested at 9:35AM and were operational. Facility files and resident medications were locked and stored in the hallway cabinets.
KITCHEN: Knives and cleaning supplies were stored inaccessible under the sink. Kitchen appliances were in good and operable condition. The facility had enough supply of perishable and non-perishable food. The freezer and refrigerator were stocked with a variety of foods and observed to be of good quality.
