Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi arrived at the facility unannounced to conduct a required annual visit. Upon arrival, LPA met with staff and explained the reason for the visit. Administrator Joji Josefa B Junio, was contacted and arrived shortly after. On 11/20/2025, at approx 12pm, LPA Martha Arroyo attempted to conduct the annual inspection, but no staff were available on site and LPA was unable to contact the Administrator. Today LPA toured the physical plant areas inside and outside to ensure there are no health and safety hazards and the facility is in compliance with Title 22 Regulations. All four (4) clients were at day programs during the visit.
At approx 10:10 a.m. LPA inspected the kitchen. Knives and sharp objects are stored in a locked cabinet underneath the sink. No cleaning supplies were observed kept underneath the sink. Kitchen appliances were observed to be in operable condition. The facility has a sufficient supply of perishable and non-perishable food properly stored at this time. A sufficient supply of PPE were observed stored in a cabinet in the kitchen. LPA observed medication to be stored in accessible in the cabinet closest to the fridge. At approx 10:20 a.m. LPA observed staff bringing in groceries.
At the time of the visit, the common area furniture's were observed to be in good condition. LPA observed multiple books and activities stored in a cabinet in the office area. The facility maintained a comfortable temperature of 72 degrees Fahrenheit. Smoke detector(s) and carbon monoxide detector were operational at the time of the visit. LPA observed fire extinguishers to be fully charged and scheduled to be service this week. LPA observed four (4) client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. The client restrooms were clean and sanitary and in operating condition. The bathrooms were sufficiently stocked with supplies and paper towels. The hot water temperature was measured in each restroom between 105 - 120 degrees Fahrenheit. Office area was located to left of front entrance.
