Facility Evaluation Report

Licensing Program Analyst (LPA) Martha Arroyo attempted to conduct an annual inspection at the above facility. Upon arrival, the LPA knocked on the front door, but there was no answer. At 12:10 p.m. and 12:15 p.m., the LPA attempted to call the Administrator; however, there was no answer. The LPA left a voicemail informing the Administrator that they were at the facility and requested a call back.

The LPA will return at a later date to complete the annual inspection.

An exit interview was conducted, and a copy of the report was emailed to the Administrator for signature.
