Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit at approx 1:30 p.m. for the purpose of investigating a self-reported incident report. Upon arrival, LPA met with Administrator Joji Junio and explained the reason for the visit.
On 04/26/2025 - Staff #1 (S1) assisted Client #1 (C1) with a shower. S1 observed a bite mark on left outer upper arm, approx 2 x 1 1/2 inches.
LPA conducted physical plant, interviewed staff and reviewed and obtained copies of pertinent documentation relevant to the investigation.
No immediate health and safety concerns were observed during the visit. LPA has determined further investigation is needed and will return at a later date if necessary .
Exit interview conducted. A copy of the report was issued.
