Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi conducted an unannounced required annual visit. At 12:44 p.m., the LPA was greeted and screened by staff. At 12:47 p.m., the LPA spoke with the Administrator over the phone. The Administrator was not available during today’s visit and authorized staff, Sarah McGary to sign the report. When the LPA arrived, there was one staff and all the clients were at day program. This home is vendored by Tri-Counties Regional Center as a level 2.
This annual had a specific emphasis on infection control practices and procedures. At 12:58 p.m., the LPA, along with staff toured the physical plant areas inside and outside to ensure there are no health and safety hazards and that the facility is in compliance with Title 22 Regulations.
BEDROOMS: There are four (4) client bedrooms and one (1) staff room. The LPA observed the client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting.
RESTROOMS: Restrooms were observed to be clean and sanitary with hand soap, toilet paper and paper towels. At 1:02 p.m., the hot water temperature tested at 109.4-degree Fahrenheit.
COMMON SPACES: At the time of the visit, living room and dining room furniture was observed to be in good condition. The LPA observed the fire extinguisher to be fully charged and last serviced on 08/06/2022. Signs are posted throughout facility to promote handwashing, and cough/sneeze etiquette. At 1:05 p.m., fire alarms and carbon monoxide detectors were tested and functioning properly. Medications are centrally stored and in a locked filing cabinet near the living room.
