Facility Evaluation Report
Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced Case Management – Incident visit at approx 09:45 a.m. for the purpose of investigating a self reported incident report. Upon arrival LPA met with Day Program Manager Jared Disbrow-Brunton and explained the reason for the visit.
On 04/23/2025, the Regional office received a self-reported SOC 341, which stated, on 04/21/2025, the family / responsible party of Client #1 (C1), was observed with a bruise on left side of C1's thigh and returned home soiled and diaper was unchanged.
LPA conducted physical plant, interviewed staff and reviewed and obtained copies of pertinent documentation relevant to the investigation.
No immediate health and safety concerns were observed during the visit. LPA has determined further investigation is needed and will return at a later date to complete the investigation if warranted.
Exit interview conducted. A copy of the report was issued.
