The inspector’s account

Licensing Program Analyst (LPA) Brian Balisi arrived at the facility unannounced to conduct a required annual visit. Upon arrival, the LPA met with Staff and explained the reason for the visit. DSP Stephen Cerbo arrived shortly after and stated the Administrators are unable to be onsite for the visit at this time, but Cerbo will sign in their place. LPA toured the physical plant areas inside and outside to ensure there are no health and safety hazards and the facility is in compliance with Title 22 Regulations. The facility is vendored by Tri-Counties Regional Center (TCRC) as a level 4-i home.

LPA inspected the kitchen/food service area at approx. 09:45 a.m. Knives and sharp objects are stored in a locked cabinet underneath the sink. No cleaning supplies were observed stored in the kitchen. Kitchen appliances were observed to be in operable condition. The facility has a sufficient supply of perishable and non-perishable food properly stored at this time.

At the time of the visit, the common area furniture's were observed to be in good condition. LPA observed medications, first aid kit, facility records stored inaccessible in hallway cabinets. LPA observed various, books, board games and activities on a shelf in the dining area. The facility maintained a comfortable temperature of 72 degrees Fahrenheit. Smoke detector(s) and carbon monoxide detector were operational at the time of the visit. LPA observed fire extinguishers to be fully charged and last purchased 03/10/2025.

All exits have functioning auditory devices and were operational at the time of the visit. There is an office area located next to the kitchen. The LPA observed required posts in this area along with a personal computer for residents to access the internet.
