Facility Evaluation Report

Licensing Program Analyst (LPA) Martha Arroyo conducted an unannounced case management – other visit at 1:00 p.m. The purpose of this visit is to obtain pertinent information regarding an incident that occurred on 12/11/2023. Upon arrival, LPA met with Program Director, Joseph Laporte and Case Manager Brianne Esseff and explained the reason for the visit. Entrance Interview.

During today’s visit, LPA Arroyo met with two staff members between 1:43 p.m. and 2:15 p.m., conducted a record review, and obtained copies of pertinent documents.

Exit interview conducted. Report was reviewed and a copy was issued.
