Facility Evaluation Report

Licensing Program Analyst (LPA) Brian Balisi conducted an unannounced visit at the facility in conjunction with investigation of complaint control #29-AS-20260212155920. Upon arrival LPA met with Administrator Myline Olivas and explained the reason for the visit.

Prior to today's visit LPA conducted facility file review, which revealed that the facility's licensee AM3 Corporation indicates a suspended status with the Secretary of State of California. Administrator stated they will have it in good standing by next month.

The following deficiencies were observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22 and California Health and Safety Code. Failure to correct the deficiencies may result in civil penalties. Exit interview conducted.
