Facility Evaluation Report

Licensing Program Analyst (LPA) Teresa Camara conducted a collateral visit to this facility regarding complaint numbers 29-AS-20241015160709, 29-AS-20240501135228, and 29-AS-20240130110843 which pertain to another licensed facility. LPA interviewed clients who live at the facility.

LPA met with the new Program Director/Administrator of Pals Skill Center, Jesus Landeros, and explained the reason for the visit. Administrator informed LPA he has been in the position for about two and half months.

During LPA's visit today, LPA interviewed two clients starting at 10:01 a.m.

No deficiencies were observed during LPA's visit. Exit interview conducted and report issued.
