Facility Evaluation Report
LPA Cortez conducted an unannounced Case management-deficiency visit to address the deficiencies noted during complaint investigation visit conducted on 10/17/25.
After review of the Licensing Information System (LIS) facility personnel report summary, LPA observed that two staff (S1-2) who were present during today's visit were both not associated to the facility and S1 does not have a fingerprint clearance. Staff interviews revealed a staff (S3) who was not listed on the Personnel record also works at the facility. S3 is also not associated to the facility. LPA verified all three staff were not associated on the Guardian website and S1 did not have the fingerprint clearance. During today's visit the LPA observed S1 with access to resident's medications, upon observation the Designee stated they and S1 were working on the medications.
Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D):
