Facility Evaluation Report

Licensing Program Analyst (LPA) Kristin Kontilis conducted an unannounced annual required visit. LPA met and toured the facility with Leilani Macasias, Administrator. The program is a non-profit Adult Day Program. The facility operation is in a leased building by the non-profit corporation 501(c)(3). The facility maintains a contract with Tri-Counties Regional Center.

The facility location is located in a business area of the community and consists of one building with multiple entrances and exits. The facility hours are Monday through Friday, 8 am to 3:00 pm. The program site is clean, safe, sanitary, and in good repair.
