Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi along with Quality Assurance Specialist (QAS) Tri-Counties Regional Center Katy Robison arrived at the facility unannounced to conduct a required annual visit at 10:45 a.m. At 11:00 a.m., the LPA and QAS met with one of the Administrators, Emmanuel Soratorio and explained the reason for the visit. The facility is vendored by Tri-Counties Regional Center as a level 3 home.
RECORD REVIEW: Between 11:17 a.m. and 1:54 p.m., the LPA conducted a file review for all six (6) clients and for five (5) staff and observed the following: Staff have current first aid and training completed. Client records were reviewed for, but not limited to current Individual Programming Plan (IPP), medical records, P & I funds, admissions agreement, and consent forms. All records were in order. The last earthquake drill took place on 05/20/2025 and fire drill on 04/10/2025. Administrator’s Certificate for Emmanuel Soratorio is valid until 09/15/2026. Administrator’s Certificate for Amalia Soratorio is valid until 10/21/2026. The LPA observed documentation of the Infection Control Plan and Emergency and Disaster Plan.
Starting at 1:55 p.m., the LPA conducted a review of medication and medication documentation with staff for three (3) out of six (6) clients and observed that medications were properly documented and assisted as prescribed.
Starting at 2:10 p.m., the LPA, along with the Administrator toured the physical plant areas inside and outside to ensure there are no health and safety hazards.
