Facility Evaluation Report

Licensing Program Analyst (LPA) Kelly Dulek, along with Tri Counties Regional Center Quality Assurance Specialist (QA) Liz Aced-Arnett conducted an unannounced Case Management - Incident inspection at the facility today to follow up on a self-reported incident report relating to Resident #1 (R1). LPA and QA met with Administrator Charles (Martin) Carino. Entrance interview conducted.

The reason for today's inspection is to follow up on the self-reported incident report received on 04/08/2026 pertaining to Resident #1 (R1). Incident report indicates that on 04/08/2026, R1 reported Staff #1 (S1) had slapped R1 on a Saturday in January.

During today's visit, LPA and QA interviewed Administrator at 02:20PM and interviewed R1 at 02:48PM. LPA and QA reviewed and obtained copies of relevant documents. Interview and record review revealed that S1 did not work at this facility on any Saturday in January 2026 or anytime prior to or after that time frame. Interview with R1 revealed the alleged incident occurred "awhile" ago, but R1 could not recall the date or the month the alleged incident occurred. R1 stated that everything is okay now. Administrator indicated the facility schedule has changed and S1 does not work at this facility at this time.

If further investigation is warranted, LPA may return at a later date.

No deficiencies cited. Exit interview and report reviewed with Licensee Representative Richard Carino. A copy of the report was provided.
