Facility Evaluation Report
Licensing Program Analysts (LPA) Esther Cortez along with Tri-Counties Quality Assurance Specialist (QAS) Liz Aced-Arnett conducted an unannounced Case Management -Deficiencies visit. The purpose of the visit is to issue citations for deficiency observed during a case-management inspection not related. The LPA met with Administrator Dadesii Daniel and explained the reason for the visit.
At approximately 9:05 a.m. the LPA and QAS observed the Administrator enter the facility which was unlocked by just opening the door. Upon observation, QAS questioned why they do not lock the home as it could be dangerous, and the Administrator revealed that they always leave the home open. During the visit, the Administrator informed staff to start locking the home.
The following deficiencies were observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22. Failure to correct the deficiencies may result in civil penalties.
Exit interview was conducted. A copy of the report and appeal rights were provided.
