Facility Evaluation Report
Licensing Program Analyst (LPA) Esther Cortez and Tri-Counties Regional Center Quality Assurance Specialist (QAS) Liz Azed- conducted a joint Case Management visit at the above facility continuing the inspection that began on 09/14/2021.
On 09/14/21, Licensing Program Analyst (LPA) JoAnn Rosales and Tri-Counties Regional Center Quality Assurance Specialist (QAS) Freddie Garcia conducted a joint Case Management visit at the facility to investigate alleged incidents potentially occurring from August 23, 2021 to August 25, 2021 where staff pushed client #1 (C1) to the ground 7 days in a row and while in the shower C1 fell and staff would not assist C1 in getting up. C1 sustained a gnash to the knee in two places due to the fall. LPA Rosales and QAS Garcia toured the facility with staff Damiana Alaniz, interviewed clients and random staff, reviewed staff and client records and requested copies of pertinent client documents. Further investigation is required.
During today's visit LPA Cortez and QAS Aced-Arnett, interviewed Administrator Didi Daniels who revealed with the exception of one other staff and herself all the staff and C1 involved in the alleged incident from 2021 no longer work or reside at the home. The Administrator further revealed that they do not recall many details from 2021. LPA reviewed interviews conducted by LPA Rosales. None of the staff or residents interviewed witness Staff 1 (S1) push C1 and multiple staff reported that C1 refused assistance at times and even during falls C1 would refuse to stand up.
No immediate health and safety concerns were observed, no further investigation is required.
Exit interview was conducted, today's report was reviewed and emailed to the Administrator.
