The inspector’s account

Licensing Program Analyst (LPA) Kelly Dulek arrived at the facility unannounced to conduct a required annual visit. At 11:00 a.m., the LPA spoke with licensee representative over the phone who indicated neither licensee nor Administrator are available today, however facility designee is authorized to sign today's report. Facility Designee Ralph Garcia arrived at 11:05AM. Entrance interview conducted.

The facility is vendored by Tri-Counties Regional Center. The facility is a 2-story home, however the upstairs remains locked. All clients reside in the downstairs area, so LPA toured the downstairs only. Beginning at 11:17 a.m., the LPA, along with the facility designee toured the physical plant areas inside and outside to ensure there are no health and safety hazards. The following was observed:

BEDROOMS: There are five (5) client bedrooms in the home; one (1) is shared and four (4) are designated for private use. LPA observed client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level.

RESTROOMS: Two (2) shared client restrooms were observed to be relatively clean and sanitary and in operating condition. Water temperature was measured in both client restrooms and measured within the required range. The restrooms had sufficient liquid soap and paper towels.

KITCHEN: The LPA observed the kitchen and dining area. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Knives are stored in a locked kitchen cabinet. Cleaning solutions and chemicals are inaccessible and locked away underneath the kitchen sink.
