Facility Evaluation Report
Licensing Program Analyst (LPA) Emily Peraldi along with Quality Assurance Specialist (QAS) Tri-Counties Regional Center Patrick Brown conducted an unannounced Case Management - Incident inspection. At 9:40 a.m., the LPA and QAS met with Administrator, Vanessa Garcia explained the reason for the visit.
The reason for today's inspection is to follow up on a self-reported incident report received on 05/11/2025. The incident report pertains to Staff #1 (S1) possibly inappropriately touched Client #1 (C1). At 11:46 a.m., the LPA and QAS conducted an interview with the Administrator. At 12:05 p.m., the LPA requested and received copies of pertinent documents.
No immediate health and safety concerns were observed during today's inspection.
