The inspector’s account

Licensing Program Analyst (LPA) Martha Arroyo conducted a Case Management - Deficiencies visit in conjunction with a complaint visit (Complaint Control # 29-AS-20260727122539). The purpose of the visit is to issue a citation for deficiencies observed during the initial complaint investigations unrelated to the complaint. During today’s visit, LPA met with Administrator Elizabeth Galang and the reason for the visit was explained.

During today's visit, it was determined that Resident #1 (R1) is bedridden. According to R1's Physician's Report, dated 12/19/2025, R1 is unable to independently transfer to and from bed and is considered bedridden. A review of the facility's license revealed that the facility has fire clearance for six non-ambulatory residents only and is not approved for any bedridden residents. The Administrator contacted R1's family to arrange for R1 to be relocated from the facility.

Pursuant to Title 22, California Code of Regulations and/or CA Health and Safety Code, the following deficiencies were cited (refer to LIC 809-D.)
