Facility Evaluation Report
Licensing Program Analyst (LPA) Melisa Rankin arrived at the facility, unannounced, to conduct the required annual visit. At 9:06 a.m., the LPA met with the Licensee, Carlos Espena and explained the reason for the visit. During tour, the Administrator Louisa M. Nolan and Licensee Julia Espena arrived at the facility. The facility is vendored by Tri-Counties Regional Center as a level 3 home.
The LPA, along with the Licensee toured the physical plant areas inside and outside to ensure there are no health and safety hazards. Facility is a single-story residence that consists of three (3) client bedrooms, two (2) staff rooms and three (3) bathrooms.
KITCHEN: The LPA observed the kitchen and dining area. Kitchen appliances are in operable condition. The facility has a sufficient supply of perishable and non-perishable food. Hot water temperature measured 110.3-degree Fahrenheit. Knives are stored in a locked kitchen drawer.
BEDROOMS: The LPA observed client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level.
RESTROOMS: Client restrooms are relatively clean and sanitary and in operating condition. The sinks had sufficient liquid soap, and paper towels.
OUTDOOR SPACE: The LPA observed the back patio which has a covered outdoor area for client use. There are no bodies of water on the premises. The laundry units are located on the back patio. Facility sketch was reviewed for gates and exits for safe evacuation and emergency response purposes, paths needing to be cleared of potted plants and residents chairs was noted and administrator will provide LPA with photo showing area updates. Back storage shed and potting cabinet on in the back yard contain gardening chemicals and tools. A lock is on both storage areas. Continued 809-C
