Facility Evaluation Report
Licensing Program Analyst (LPA) Melisa Rankin arrived at the facility, unannounced to conduct a required annual visit. At 8:17 a.m., the LPA met with administrator Rebecca Hernandez and explained the reason for the visit. At 9:20 a.m., co-Administrator, Vanessa Garcia arrived at the facility and resumed the last half of the visit. The facility is vendored by Tri-Counties Regional Center as a level 4i home.
The LPA along with Rebecca Hernandez at approximately 8:20 a.m., toured the physical plant areas inside and outside to ensure there are no health and safety hazards.
MEDICATION: Starting at 8:50 a.m., the LPA conducted a review of medication and medication documentation and observed that medications were properly documented, no medication was expired and all medication was documented on a Medication Administration Record and on a Centrally Stored Medication and Destruction Record.
BEDROOMS: The LPA observed client bedrooms, which were furnished appropriately with clean linens, appropriate furnishings and sufficient lighting. Inside temperature was maintained at a comfortable level.
RESTROOMS: Client restrooms are clean and sanitary and in operating condition. The sinks had sufficient liquid soap, and paper towels.
OUTDOOR SPACE: The LPA observed the back patio which has a covered area equipped with furniture for client use. There are no bodies of water on the premises. There are two (2) side gates for emergencies.
