Facility Evaluation Report

Licensing Program Analyst (LPA) Esther Cortez conducted an unannounced Case Management - Annual Continuation at the facility today continuing the inspection that began on 09/08/2025. The LPA met with Licensee Perfecto Cudal and explained the reason for the visit.

The LPA advised the Licensee that on file with the department it indicated they were also the Administrator. The Licensee advised the LPA that the Administrator of the facility is Dennis Cudal. The LPA advised the Licensee to submit a change of administrator packet with all the required documents to make the change.

Record Review: A review of facility files was initiated. Facility records are stored in the locked staff room. The LPA observed documentation of Infection Control, Emergency Disaster Plan, Insurance liability and last Disaster drill (conducted on 07/2/2025). The LPA obtained Client Roster, and Staff Roster. The LPA reviewed five (5) out of five (5) client files. The LPA observed four (4) out of four (4) staff files. The LPA identified that Administrator Dennis Cudal last HIV and TB training on file was for 02/22/11, more than the required updated training of every two years. Otherwise all documents reviewed appeared complete and current.

Medications: LPA Conducted a medication review for three (3) out of five (5) residents. Medications are centrally stored and stored in a locked staff room; medications are labeled and checked for expiration dates. Medications are properly documented on the centrally stored medications and destruction record.

Pursuant to Title 22 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D): Exit interview conducted and copy of the report and appeal rights provided to Licensee.
