Facility Evaluation Report

Licensing Program Analyst (LPA) Esther Cortez conducted a case management - incident visit. The LPA met with staff Angela Blue-Heil and explained the reason for the visit. Administrator Floro Cortes could not be available during today's visit and designated staff Angela Blue-Heil to review and sign the report.

This visit is regarding an incident which took place on 11/26/2024 between client 1 (C1) and client 2 (C2).

During today's visit the LPA conducted two (2) staff interviews. All of the current clients were at their Day Program.

Further investigation is needed regarding this incident. No citations were issued. Exit interview conducted and report provided.
